Charter Oak Hospital offers compassionate and individualized treatment for mental health and substance use disorders. We believe in a patient-centric approach, where every treatment plan is tailored to meet the specific needs of each person who walks through our doors.
What You’ll Do:
The Case Manager provides quality case management, psychotherapeutic services, and discharge planning to all patient and their families; to serve as a member of interdisciplinary team supporting the organization’s treatment program and philosophy and assure the deliverance of quality treatment to patients and their families.
License/Certification:
- Prefer – ASW, AMFT, APCC
- Required – Current BLS for healthcare provider card upon hire
Education & Experience:
- Bachelor’s Degree in Social Work, Psychology or Social Services
- Master’s Degree preferred
- 3 years behavioral health experience preferred
- Associate license such as ASW (Associate Clinical Social Worker), AMFT (Associate Marriage & Family Therapist), APCC (Associate Professional Clinical Counselor) preferred
- Current BLS for healthcare provider card required upon hire
- Therapeutic Options™ training to be obtained prior to providing direct patient care without a preceptor
