Position Overview
The Care Navigator plays a key role in supporting patients by helping them navigate the healthcare system, access resources, and stay engaged in their care. This position works closely with clinical teams, patients, and families to improve care coordination, patient experience, and continuity of services.
Responsibilities

Serve as a point of contact for patients and families throughout their care journey

Assist patients with understanding appointments, referrals, and next steps in care

Support care coordination between departments and healthcare providers

Provide patient education and resource navigation

Conduct follow-up outreach to patients as needed

Assist with documentation and data tracking in electronic systems

Identify barriers to care and escalate concerns to appropriate team members

Maintain patient confidentiality and comply with HIPAA regulations

Collaborate with multidisciplinary healthcare teams
Qualifications

Minimum age requirement: 18 years old

Ability to commit to required shifts per month

Active health care insurance through out program participation
- Minimum 2.5 GPA

Must successfully complete background check, drug screening, health clearance, and onboarding requirements
